Guidelines for Surgery at the Hospital

Anaesthetic Rooms

Registration Form
The forms in the Hospital pack MUST be completed and returned
to the Hospital at least one (1) week prior to your surgery
Green Consent Form
The surgeon will complete this form at the time of your
consultation where it will be signed by you and given to your
reception.
This form will need to be at our Rooms prior to your admission.
Instructions
PLEASE DO NOT EAT OR DRINK ANY
FLUIDS FOR SIX (6) HOURS PRIOR TO THE PROCEDURE.
(Your meal prior to surgery should be a light one)
Surgeon's Fee
Payment prior to or on the day of your procedure is required.
